Entre 1984 y 1986, como estudiante de medicina en la Universidad de Chile, presidió el Centro de Alumnos de su Facultad, fundó el movimiento ecologista "Pehuén" y fue dirigente de la Federación de Estudiantes de la Universidad de Chile. Político del Partido por la Democracia, el fue Senador por la 7ª Circunscripción, Región Metropolitana, período 2014-2022 y Senador por la 7ª Circunscripción Santiago Poniente, Región Metropolitana, período 2006-2014 y 2014 al 2018. Girardi fue Presidente del Senado entre 15 de marzo del 2011 al 20 de marzo de 2012 y Vicepresidente del Senado entre el 22 de marzo de 2017 y el 11 de marzo de 2018. En junio de 2019, la Organización de las Naciones Unidad para la Alimentación y la Agricultura (FAO) le otorgó el Premio Jacques Diouf 2018-2019 por su "notable contribución a la seguridad alimentaria mundial". Ese mismo año, la Universidad de Chile lo reconoció por sus aportes a la promoción de la alimentación saludable.
